HOW TO REGISTER
1. Use one registration form per attendee. Photocopy as
necessary.
2. Fill out business information. Be sure to include your FAX
number and your SAI account number to obtain dealer pricing.
3. Check the course(s) you are registering for: Truform•OTC
orthotics only, Airway mastectomy only, or both. Attendance at the
Acor footwear program requires a separate registration form and
payment. NOTE: The Airway and Acor programs are given at the same
time, so it is not possible to take both at the same site.
4. Payment must accompany your registration. Mark the payment
method selected, and fill in the pricing for the course(s) you have
chosen. We accept VISA and MasterCard. If paying by credit card, print
clearly the card number, expiration date, and name appearing on the
card.
5. You will receive a fax confirmation within five (5) business
days after receipt of your registration.
